Description
Individuals living with mental illness who are incarcerated at Niagara County Jail and who will be discharged to Niagara County may receive discharge referrals and linkages. For the highest acuity cases, a limited amount of temporary case management is available to assist with linkages to community based entitlements and outpatient providers upon discharge.

Monday-Friday, 8:30am-4:30pm.
