Case/Care Management
Description
Provides care management services for children, including in the following areas:
- Appointment scheduling
- Transportation to and from medical appointments
- Patient and family support (communication between care providers, connections to needed services, access to resources, advocate services, etc.)
- Educational support (help with school resources; ex. tutoring, special education services, IEP meetings, etc.)
- Community referrals and support (housing, food, summer camp, etc.)
- Interpretation services (Spanish and American Sign Language)

By appointment; call to schedule.
Eligibility
Children ages 3-21 whose families are Medicaid eligible or have active Medicaid, and who meet the following conditions:
- Have two or more chronic conditions (ex. Asthma, obesity, GERT)
- OR have a single qualifying condition of either HIV /AIDS, Serious Emotional Disturbance (SED), or Complex Trauma
- In addition, an individual must be appropriate for Health Homes services (due to behavioral, medical, or social risk factors)
