Description
Care coordinators assist clients with:
- Finding a doctor or specialist
- Making sure that doctors are talking to each other and the client about their care
- Advocating for the client
- Helping the client understand and remember confusing information
- Scheduling appointments, tests, and more
- Getting care when the client is discharged from the hospital
- Getting into mental health counseling or substance use programs
- Finding transportation to medical appointments

Monday-Friday, 9am-5pm.
