Case/Care Management
Description
Provides family-driven care management services, including Comprehensive Care Management, Care Coordination, Health Promotion, Comprehensive Transitional Care, Individual and Family Support, and Referral to Community and Social Support Services.
Hours
Monday-Friday, 8:30am-4:30pm.
Eligibility
Children with active Medicaid who have 2 chronic conditions or a diagnosis of serious emotional disability (SED).
