Case/Care Management

Description
A Health Home isn't a place, but a service that provides free care management services to Medicaid recipients to ensure that their medical, mental health, and substance use disorder needs are being met. BestSelf Health Home Services work to ensure that everyone involved in an individual's care is working cooperatively and sharing the information necessary to support recovery. In addition to physical health services, the Health Home assists in coordinating social service needs.

Hours
Monday-Friday, 8:30am-4:30pm.

Eligibility
Medicaid recipients 18 or older living in Erie County, including those who are already members of a Managed Care Organization or have both Medicaid and Medicare.

Must meet one of the following criteria to become a BestSelf Adult Health Home member:
- Have two or more chronic health conditions such as asthma, diabetes, heart disease, mental health, or substance use disorders
- Have a significant mental illness
- Living with HIV/AIDS
