Chronic Disease Self Management Programs

Description
Individual, group, and Telehealth diabetes self-management education support services provided to meet the personal needs and goals of individuals with diabetes, whether newly diagnosed or having long-term diabetes. Program is "ADA Recognized," meeting very strict ADA National Diabetes Self-Management Education and Support standards. Faculty members are Certified Diabetes Educators.

Hours
Monday-Friday, 8am-4pm. Flexible scheduling if needed; classes available on weekends.

Eligibility
Self-referral, physician referral, or referral by another healthcare professional.

Service Area: Southern Tier of WNY and Northern Pennsylvania.

Program Fees
There are fees for services, but most insurances cover part or all of the cost. You may be eligible for financial aid.

Languages
All languages can be accommodated through Interpreter Services, including for those who are hearing impaired.
