A separate appointment will be required for each year being filed for. Appointments to amend a return must be booked for after March 1st, 2024.

Must complete Form 13614-C prior to the appointment. Without the form completed in advance, tax preparers will be unable to start the appointment on time. The form can be picked up at the Northwest Buffalo Community Center, Room 113, or downloaded and printed online at: https://freetaxbuffalo.com/docs/appt-confirmation-mail.pdf

1. Be on time to your appointment but do NOT arrive early; you will not be allowed in until your appointed time.
2. Enter the building from the doors in the rear of the building (the door with the ramp)
3. Bring your own pen.
4. Persons other than the taxpayer(s) will not be permitted in the building with you. Please leave children and other relatives at home.

Please bring the following to your appointment:
- Social Security cards for you, your spouse, and/or dependents
- Proof of identification for yourself and/or your spouse (driver's license)
- A copy of last year's tax return - VERY IMPORTANT
- Bank account information for the direct deposit of your refund (routing and account number)
- Copy of IRS Identity Theft PIN Letter, if you received one - IMPORTANT
- Copies of any letters/notices from the IRS or NY State

Bring the following as they apply:
- ALL Wage Statements (W-2) from your employer - if not received, you must contact the employer
- Information for all other income (ex: Self-employment / Lyft / Uber / Translation)
- Information for all deductions/credits
- Total paid to a day care provider and their tax ID number
- Forms 1095-A, B or C (Health care /ACA Statements)
- ANY FORMS 1099
