Homeless Permanent Supportive Housing

Description
Permanent supportive housing for chronically homeless women in a single site utilizing the housing first approach. Residents/clients are provided low demand support services including counseling, case management, job development, and assistance with daily living. Breakfast and dinner served daily. All referrals are accepted through coordinated entry; see Intake below for more details.

Hours
Monday-Friday, 8:30am-5:30pm.

Eligibility
Must be an unaccompanied adult female with a disabling condition and homeless for one year or more, or have 4 homeless episodes in a 3 year period.

Service Area: Erie County.

Intake Process
Coordinated entry system, meant to streamline the process for targeting housing for individuals and families who are homeless in Erie County.

Coordinated Entry Locations (in-person):
- The Community HUB location in this Continuum of Care is the Harbor House Resource Center at 241 Genesee Street in the City of Buffalo. This location can be contacted at 716-842-4184 ext. 131. The Coordinated Entry HUB will be open as follows: Saturday & Sunday, 9pm-7am and Monday-Friday, 9pm-3pm.
- A Resource Room is located in the basement of the Buffalo and Erie County Library Central location at 1 Lafayette Square, Buffalo , NY 14203. This location can be contacted at 716-367-8188. The Library Resource Room will be open Monday-Friday, 8:30am-4:30pm.
