Description
Helps people get health insurance for themselves and their families and offers a convenient way for people to learn about available health insurance programs in their community.

Facilitated Enrollers are trained as experts to screen and determine eligibility for all government insurance programs. They will help the patient gather the necessary documentation and represent the patient at the Department of Social Services. Facilitated Enrollers can help patients apply for Medicaid, Child Health Plus, Prenatal Care Assistance Program (PCAP), and State Aid.

Hours
Monday-Friday, 8am-7pm.

Eligibility
Individuals who have low or no income and need assistance in obtaining Medicaid or Premium Tax Credit towards purchasing a health insurance plan through the NYSDOH Marketplace.

Service Area: Erie County.

Program Fees
Enrollment and counseling are free. Cost of insurance will depend on the individual.
