PACE Programs
Description
Provides preventive services allowing individuals to stay in their homes and out of nursing home care. Provides socialization by way of a Day Center, which also houses both a Medical Clinic and a Physical and Occupational Therapy Clinic. Offers home care, personal care, primary and specialty physician services, prescription drugs, meals (both in the Day Center and delivered to the home), durable medical equipment, and round trip transportation to the Day Center and to all medical appointments.

Monday-Friday, 8am-4pm.
Eligibility
Niagara County residents ages 55 and older who are nursing home eligible as determined by a nurse's assessment.
