Description
A residential service for persons who have been diagnosed with serious mental illness. The program places these individuals with community members certified to deliver residential care in their own homes. BPC staff supervise the Family Care Home and provide case management services for the residents. The home provides meals, housekeeping, medication administration assistance, and support with day to day stressors and routines.

Hours
24 hours a day, 7 days a week.

Eligibility
Adults 18 and over who have a serious persistent mental illness (SPMI) and are in need of community placement. Should be able to ambulate (walk) independently, demonstrate a commitment to recovery and willingness to take medications as prescribed.

Exclusions for placement include sex offender status, dementia diagnosis and/or cognitive impairment, pattern of wandering and/or leaving without notice, history of violence or self-abuse.

Service Area: Cattaraugus, Chautauqua, Erie, & Niagara Counties.

Intake Process
Staff will evaluate for appropriate admission criteria and determine if there is a Family Care Home that would meet the individual's needs and be a good fit.
