Early Identification Programs
Description
Early identification and referral program for children ages birth to 3 years who are at risk for developmental delay. Services include home visits as necessary, providing guidance about the child's growth and development as well as developmental screenings and assessment if this is not being provided by the family's doctor. Also provides assistance applying for insurance or finding a doctor.
Hours
Monday-Friday, 8:30am-5pm.
Eligibility
Premature or low birth weight infants; infants who spent 10 or more days in NICU; infants of teen mothers; infants with congenital anomalies or inherited metabolic disorders; infants whose mothers engaged in prenatal substance abuse; infants with a diagnosed condition or risk factor; any infant with a suspected developmental delay or parental concern; children without insurance; children without a primary care provider.
