Description
Provide free information and assistance on long term care service options for people of all ages who are disabled and/or chronically ill, as well as the frail elderly residing in Wyoming County.
Serves as the point of contact for the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs.

Monday-Friday, 8am-5pm.

Any person of any age who is chronically ill or disabled, as well as the frail elderly, and their caregivers.
