Description
Strengthens the capacity of families to nurture, support, and provide for their child's academic development, emotional growth, and physical well-being by establishing effective relationships between school and home. Home visitors offer individualized, interactive early learning experiences for the young siblings of school-aged children. During monthly home visits, parents engage in fun-filled and developmentally appropriate activities with their child. The program also provides opportunities for Ages and Stages Questionnaire (ASQ-3) developmental screenings, Ages and Stages Social Emotional screening (ASQ-SE), group activities, and connections to community resources.

Offers the following programs, using the evidence-based Parents as Teachers and Healthy Families Home Visiting Models:

- PAT (Parents As Teachers): Home visiting / early childhood and family support program
- PACERS (Parents Activating Children's Education Results in Success): Elementary school home visiting program in all Jamestown Public Elementary Schools.
- EASE (Educational Advocacy & Scholastic Enrichment): Middle school home visiting program in all Jamestown Public Middle Schools to help "EASE" the transition from elementary school to middle and high school.
- ASA (After School Amigos): After school and family engagement program for elementary school age English Language Learners at Love and Ring Schools.
- TEAM Program (Teenage Education and Motherhood): Supports pregnant and parenting adolescents who are working to obtain a high school diploma.
- PACT (Parents And Children Together): Inter-generational pre-K program at Love School. Emphasizes parent engagement in the classroom.
- Community Based Program Parents as Teachers: Utilizes the Parents As Teachers program with families who live in Chautauqua County.
- Healthy Families Chautauqua Program: Supports families with pregnancies or with a child under 3 months of age living in Chautauqua County.

Hours
Monday-Friday, 8am-4pm.

Eligibility
Families in Chautauqua County with children who are prenatal to 8th grade.

Program Fees
Free.

Languages
English and Spanish speaking home-visitors are available; however, English classes are not offered.
