Services offered include: home health nursing and aide care, service coordination, independent living skills training, positive behavioral interventions, peer mentoring, community integration counseling, home and community support services, and structured day program services. Provides specialized programs for: Nursing Home Transition and Diversion, Alzheimer's and dementia, Traumatic brain injury, stroke, chronic illness, and disability care.

Eligibility
Individuals 18 years of age and up who are Medicaid eligible. Also accepts long term care insurance, workers' compensation, and private pay.
