Provides dental care, vision care, primary care, pediatric care, and behavioral health care in a Certified Patient Centered Medical Home that is accessible and focused on quality and safety. Also offers health insurance navigation services.
Hours
Monday-Thursday, 8am-8pm; Friday, 8am-5pm; Saturday, 8am-12 noon.
Eligibility
Open to all members of the public.

Call to schedule an appointment or walk in.
Program Fees
Accepts private health insurance and managed care plans, public health insurance (Medicare, Medicaid, Child Health Plus), cash, check, or credit/debit card. Sliding fee scale available. Will provide assistance with enrolling in health insurance coverage.
