Description
Services offered include outreach services, help with Medicare, SNAP, HEAP, and tax forms, telephone assurance program, nutrition program, home visits,, senior transportation, health screening, education, recreational/cultural programs, caregiver training/support, educational and fitness classes and support groups.
Erie County residents are eligible to participate in the Stay-Fit Dining Program and Frozen Meals Program.
Social caseworkers provides information and referral for long term care, counseling for caregivers looking for long term care options, and assistance to eligible individuals with obtaining funding for home care, social daycare, managed long-term care, etc.
Serves as a Cooling Center for individuals during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.

Monday, 8:30am-7pm; Tuesday & Thursday, 8:30am-7:30pm; Wednesday & Friday, 8:30am-4:30pm.
