Description
A community-based support service for adults throughout Erie County with serious and persistent mental illness and adults with Medicaid that have two chronic health conditions. Provides individual service planning with the following services available to those receiving services: crisis intervention on a 24-hour basis; linkages to medical, psychiatric, social, educational, psychological, and vocational services within the community; coupled with individual and systemic advocacy.
Hours
Monday-Friday, 9am-5pm.
Eligibility
Individuals 18 and older with serious and persistent mental illness or individuals with Medicaid that have 2 chronic health conditions; call for specific information.
