Description
Community based case management services for adults with mental illness living in the community independently, with family, or in community residences.
Offers three levels of mental health case management to meet the varying needs of consumers: Intensive Case Management (ICM), Supportive Case Management (SCM), and Community Support Services (CSS).
Provides a continuum of care with maximum flexibility and the option to easily and efficiently move from one level to the next as a consumer's needs change.
