Description
Provides care coordination, specialty and primary health care, HIV/AIDS specialty care, LGBTQ+ specialty care, STI/hepatitis C testing, syringe service program, buprenorphine program, and harm reduction program.
Also provides linkage to mental health services, substance use counseling, housing assistance, nutrition assistance, and transportation assistance.

Hours
Monday-Friday, 8:30am-4:30pm.
