Description
Comprehensive residential substance use disorder treatment services designed to assist individuals to maintain abstinence from all mood altering substances, to be successfully reintegrated into their communities, and to continue effective community adjustment. Suboxone treatment available for enrolled patients.

Program Fees
Accepts most commercial health insurance plans, Medicaid, and Medicare. Insurances accepted vary based on service type provided, such as outpatient treatment vs. inpatient and residential treatment. Sliding fee schedules are available for eligible patients and are based on income level and family size. Horizon serves all patients regardless of ability to pay.
