Geriatric Counseling
Description
Home-based assessment and psychotherapy program designed to meet the mental health needs of seniors who are at risk for out of home placement. Provides a psychological interview and assessment, treatment planning, and subsequent psychotherapy sessions conducted by a licensed psychiatric social worker, typically in the client's home or in the Barker Street office.
Program is intended to treat psychiatric disorders such as depression, anxiety, etc. Medication management is provided for GAP clients engaged in treatment in the program who are able to be seen at the agency by the psychiatrist or psychiatric nurse practitioner. Linkages are also provided for case management and socialization programs.

Hours
Monday-Friday, 9am-4:30pm.
Eligibility
Adults ages 60 and older with a mental health diagnosis, excluding those with a dementia diagnosis.
