Adult Day Health Programs

Description
Medical Adult Day Health Care Program that services people who are visually impaired and medically compromised. Meals provided. Transportation is arranged. Provides case management, both medical and social for participants.

Clinical services available to participants may include: audiology (referral basis), referral to dentist and podiatry, RNs on staff, and registered dietician. Occupational therapy, physical therapy, and speech language pathology are provided. Social worker on staff. Vision rehabilitation and training provided.

Teaches skills for daily living for people who are visually impaired, such as adaptive handling of money, Braille, food preparation (peeling, slicing), mobility training, and safe use of stairs. Recreation therapy provided; includes crafting, outings, discussion groups, exercise, special events and entertainment, religious activities, and cooking groups.

Hours
Monday-Friday, 8am-4pm; two Saturdays per month, 8am-3:30pm.

Eligibility
Visually impaired and medically compromised individuals, ages 18 and older.

Service Area: Erie County.

Program Fees
Medicaid, Managed Medicaid Plans, VA, and private pay accepted.
