Description
Offers 68 scattered site, furnished apartments to chronically homeless individuals as part of a Housing First Model. The Housing First Model does not put conditions on housing; treatment and sobriety are not conditions, and income and ID are not required.

Low to medium demand supportive case management is provided. The resident will be assisted with obtaining food, appropriate clothing, identification, benefits, medical services, and follow ups. Opportunities for work activities to develop skills and income potential are made available.

Hours
Monday-Friday, 8am-5:30pm.

Eligibility
Coordinated Entry uses the HUD definition of homelessness. A “literally homeless person” is someone:
- Living in a place not meant for human habitation
- Living in emergency shelter (including domestic violence shelter)
- Living in transitional housing for homeless persons, with an imminent exit date
OR
- Exiting an institution where they temporarily resided for up to 90 days (and were in a shelter or a place not meant for human habitation immediately prior to entering that institution)

Intake Process
Intakes can occur via phone call or in-person at one of the locations below:

- The Community HUB location in this Continuum of Care is the Harbor House Resource Center at 241 Genesee Street in the City of Buffalo, 14204. This location can be contacted at 716-842-4184 ext. 131, and is open as follows: Monday-Friday, 9pm-3pm; Saturday & Sunday, 9pm-7am.
- A Resource Room is located in the basement of the Buffalo and Erie County Library Central location at 1 Lafayette Square, Buffalo , NY 14203. This location can be contacted at 716-367-8188.
