Adult Day Health Programs

Description
Provides comprehensive outpatient health and rehabilitation care, as well as a supportive group environment with recreational activities.

Health services provided include nursing care, personal care services, audiology, speech therapy, health monitoring, nutritional services, occupational therapy, physical therapy, case management, and health care monitoring of blood pressure, blood sugar levels, medication, and other health regimes.

The following services may be arranged as needed: laboratory services, radiology, dentistry, ophthalmology, podiatry, and beauty/barber shop visits.

Offers services to individuals with Traumatic Brain Injury, Multiple Sclerosis, Strokes, Parkinson's Disease, Diabetes, Hypertension, Intellectual Disabilities, and Behavioral Health issues.

Hours
2 sessions Monday-Friday, 8am-1pm & 1:30pm-6:30pm; 1 session Saturday, 11am-4pm.

Eligibility
A participant must require at least 5 hours of health services for at least one visit per week.

Service Area: Erie & Niagara Counties.

Program Fees
Accepts traditional Medicaid, Managed Medicaid, Managed Long Term Care Plans, and private pay.
