Description
A community-based program that works with individuals with chronic physical, mental health, or addictions needs. Care Coordinators help with linkages and advocacy to
assist individuals with reaching their goals and achieving success in the community. A Care Coordinator can assist with referrals to treatment, primary medical care, specialists, housing, transportation, education, vocational services, and peer services, to name a few.

Monday-Friday, 8am-4:30pm.
Eligibility
Individuals with chronic physical, mental health, or addictions needs, insured by Medicaid.
