Transition Services for Students With Disabilities

Description
To meet the needs of students with multiple disabilities, SNAP provides students a small class milieu in a Total Communication, Language Rich Environment (LRE). Instruction, adapted from the New York State Education Department standards, is provided using Total Communication - spoken, signed and written language - to meet students' individual communication needs. Student progress is assessed, pending eligibility, by the New York State Alternative Assessment (NYSAA).

The goals of the program are to prepare students to care for themselves; to develop social awareness and social skills which will enable the individual to function as social beings in the community; to achieve emotional growth which will enable the individual to maintain the lifestyle of choice; to develop motor skills; to develop personal and home management skills required for self sufficiency; to select and prepare for an occupation; to develop intellectual skills which will lead to responsible and independent citizenship; and to develop the ability to use leisure time well and in a way that will enhance creativity and enjoyment of life.

Related services, including Speech, Physical and Occupational Therapies, as well as counseling are available. Audiological services such as hearing tests and minor device repair (hearing aids, cochlear implants, etc.) are provided on-site by licensed audiologists. Adapted physical education is also available.

A therapeutic after school program is available, as is a 5 day residential program. Emphasis is placed on Daily Living Skills and transition from school to adult life.

Hours
Monday-Friday, 8am-3pm.

Eligibility
Children ages 3-21 years old who need a Language Rich Total Communication Environment due to multiple disabilities.

Intake Process
Tour the school. Apply to the State Education Department. Discuss placement at Committee on Preschool/School Education (CPSE/CSE) with your school district's Local Education Agency (LEA).
