Description
Provides nursing, social services, home health aides, social workers, infusion therapy, home health evaluations and assessments, nutritional counseling, arranging of medical equipment and supplies, personal care/homemaker services, adult day care, and physical, occupational, and speech therapy.
Long term home health care program available.
Specialized programs available in pediatrics, behavioral health, and wound care.
In-facility assessments for discharge planning at hospitals and skilled nursing facilities available upon request.
Serves as a state provider in the MOMS (Medicaid Obstetrical & Maternal Services) Program, providing comprehensive support nursing services to prenatal and postpartum women.
