Description
Provides primary and specialty care, care coordination, individual and group counseling, health education, self-management training and treatment adherence for adolescents ages 13-24 years of age.

Team specializes in care for youth and young adults who are living with HIV or at risk for HIV, sexually transmitted infections (STIs), who are experiencing homelessness, mental health and/or substance use. Also provides comprehensive and supportive service for youth who identify LGBTQ.

Hours
Monday to Friday, 8:30am-4:30pm.

Eligibility
Specializes in serving youth and young adults who are experiencing homelessness, have HIV, use substances, and/or mental health diagnosis.

Intake Process
Call for an appointment, walk-in, or referrals.

Program Fees
All insured and uninsured individuals; assistance with insurance eligibility available onsite.

Languages
Language line assistance available for any person.
