Description
A non-residential program serving individuals with disabilities in Chautauqua County. The purpose is to provide services that will assist individuals with severe disabilities to reach maximum independence and an enriched quality of life.
Services include:
- Advocacy
- Assistance with applying for SSI/SSD benefits and representation at hearings
- Assistance with applying for Community Medicaid, Medicaid Buy-In for Working People with Disabilities, Medicaid Spend-down, Managed Long Term Care, and Nursing Home/Chronic Care coverage
- Assistance with enrolling in Medicare Part D plans and applying for Extra Help subsidy
- Benefits advisement for working participants receiving SSI and/or SSD and referred by ACCES-VR
- Transition to community based living arrangements
- Home modification consultation
- Independent living skills
- Information and referral
- Peer support for individuals with developmental disabilities of all ages
- Loan closet
Free transportation is available to persons with developmental/intellectual disabilities living with family members on nights and weekends. Must have documented proof of developmental/intellectual disability; if intellectually disabled, must submit psychological evaluation.

Hours
Monday-Friday, 8am-4:30pm.
Eligibility
Any resident with a disability. Some programs have additional eligibility criteria.
Service Area: Chautauqua County, with some limited Social Security and benefits advisement services available in Cattaraugus and Allegany Counties; call for details.

Intake Process
Referral by family, agency, or self referral. Phone or zoom interview with service coordinator. Due to COVID-19, appointments must be made; unable to accommodate individuals without appointments at this time.
Wheelchair Accessible Transportation services are available to agency participants Monday-Friday.

Program Fees
Fees apply to Social Security assistance to obtain cash benefits, wheelchair transportation, and some but not all loan closet items.
Languages
Spanish, by appointment only with an SILC staff member who will arrange and schedule with an interpreter.
