Intake Process
Provides appointments that do not require a medical exam. Please arrive 15 minutes prior to scheduled appointment to check in and complete paperwork.
Childcare services not provided; please make childcare arrangements; only infants and toddlers in a car seat or stroller are allowed into the exam room.

No one will go without care based on their ability to pay.
Medicaid and most insurances accepted. Uninsured individuals may qualify for a state-funded program or a lower fee scale; for documents needed please contact the Health Center.
The following forms of payment are also accepted: cash, checks, major credit/debit cards, and money orders. Fees for services are based on household income; bring proof of income. Payment is expected at time of service; payment plans are available for many services.
