Description
A community-based organization that provides a variety of programs and services, including:

- Health Insurance Navigators who help facilitate health insurance enrollment for uninsured individuals, children, and families

- Caregiver Tech Solutions & E-Respite Coaching Workshop to support caregivers. This workshop helps explore and customize technology solutions to increase respite for caregivers; more information can be found at: www.caregivertechsolutions.org

- Strong Starts Chautauqua (SSC): A network of organizations and partners dedicated to building bright futures for Chautauqua County families and children, prenatal through age 5. Supports the county's young children and their families through community education and outreach, information and referral, maternal and early childhood risk screening, and connection to resources; more information can be found at: www.strongstartschaut.com

- Creating Healthy Schools & Communities: Dedicated to helping schools and their surrounding communities to build cultures of wellness by increasing opportunities for physical activity and healthy eating; more information can be found at: www.e1b.org/en/instructional-support/creating-healthy-schools-and-communities.aspx

- Perinatal & Infant Community Health Collaborative (PICHC): A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health.

Hours
Monday-Thursday, 8am-4:30pm; Friday, 8am-1pm.

Eligibility
- Health Insurance Navigators: Anyone under 64 years old.

- Caregiver Tech Solutions & E-Respite Coaching Workshop: Caregivers (18 and older) and their loved ones (55 and older) who live in Cattaraugus or Wyoming County.

- Creating Healthy Schools & Communities: Eligibility based on NYSDOH requirements; school districts and surrounding areas for Springville-Griffith Institute, Randolph Central School, Salamanca City School District, and West Valley School District.

- Strong Starts Chautauqua: Young children and their families in Chautauqua County.

- PICHC: Women in Chautauqua County ages 14-45 who are pregnant or who have a child under the age of 2 and qualify for social service programs.

Service Area: Allegany, Cattaraugus, Chautauqua, Southern Erie, & Wyoming Counties.
