Description
Support agency for survivors of brain injuries, disabilities, and cognitive disorders, including veterans and seniors, as well as their families. Provides help through information, education, referrals, and peer support, while promoting and advocating for safety and prevention.

Serves as the Regional Resource Center contracted by the NYS DOH for both the Traumatic Brain Injury (TBI) waiver and the Nursing Home Transition and Diversion (NHTD) waiver. Waivers may be provided to nursing home eligible individuals who have Medicaid and meet other eligibility requirements.

Also offers a Traumatic Brain Injury (TBI) Survivors Peer Support Group, which meets virtually the 1st & 3rd Wednesday of each month, 2pm-3pm.

Eligibility
Open to all. NHTD and TBI Waiver programs have specific eligibility criteria that can be found online or by calling directly.

Service Area: Erie, Niagara, Orleans, Wyoming, Chautauqua, and Cattaraugus.

Intake Process
Call for phone discussion and/or visit website. Intake forms for TBI and NHTD waivers available on website.
