Description
Support agency for survivors of brain injury and cognitive disorders, including veterans and seniors, as well as their families. Provides help through information, education, referrals, and peer support, while promoting and advocating for safety and prevention.
Serves as the Regional Resource Center contracted by the NYS DOH for both the Traumatic Brain Injury (TBI) waiver and the Nursing Home Transition and Diversion (NHTD) waiver. Waivers may be provided to nursing home eligible individuals who have Medicaid, along with additional supports to leave or stay out of a nursing home.
Also offers a Traumatic Brain Injury (TBI) Survivors Peer Support Group, which meets virtually the 1st & 3rd Wednesday of each month, 2pm-3pm.

Eligibility
Primary focus is on those with brain injury and cognitive disabilities.
Waiver applicants must be nursing home eligible and have Medicaid. TBI waiver applicants must be between ages 18-64 and have documentation of brain injury. NHTD Waiver applicants must be at least 18 years old with disabling issues that cannot be primarily related to mental health.
