Assisted Living Facilities
Description
120 bed assisted living community licensed by the NYS Department of Health for older adults who need anywhere from supervision to physical assistance to maintain their activities of daily living. 25 beds are reserved for residents with Medicaid, and the remaining beds are for private pay residents. Secure memory care unit for up to 24 residents with dementia, private pay only. Rooms include a bathroom and can be private or shared/ semi-private. Rooms come furnished. All meals are provided. Medication Management is provided.
Hours
24 hours a day, 7 days a week.
Eligibility
Adults at least 65 years old who are able to walk a minimum of 150 feet, independent with transfers, bed mobility, oxygen and diabetic care.
Intake Process
Physical and medical exam performed by a physician; must have PPD/tuberculosis test.
Program Fees
Medicaid accepted for only 25 beds in the assisted living residence. Medicaid is not accepted in memory care. Private pay rates as follows: assisted living semi-private room is $3620 per month, private room is $5135 per month; memory care unit semi-private room is $5100 per month, private room is $6750 per month. All care needs are included in the cost. No community/ admission fee.
