Provides health care services including: - Primary care, physical exams, immunizations - HIV/AIDS, hepatitis C, and STI prevention, testing, and treatment - Specialty in LGBTQ+ and transgender care - Chronic disease management services (HIV/AIDS, diabetes, asthma, obesity, etc.) - Medical care coordination - Outpatient mental health counseling - Substance use services and syringe exchange program - Pharmacy Provides supportive services including: - Drop-in center (shower, laundry, food, groups) - Financial counseling services - Nutrition education services and food pantry - Housing and transportation assistance - Support groups and group counseling See separate listings for specific services.
