Offers comprehensive medical, emotional, and spiritual support to individuals with advanced illness and their families. Services provided in home as well as at assisted living facilities, adult homes, nursing facilities, group homes, and if needed, Hospice Inpatient Unit in Cheektowaga.

Hours
Monday-Friday, 8am-4pm; on call 24 hours a day, 7 days a week.
Eligibility
All ages and diagnoses in Erie County.
