Provides nursing, home health aides, speech therapy, physical therapy, occupational therapy, social work services, home intravenous therapy, nutrition, maternal/child health, dietitian-nutrition counseling, spiritual care, and Personal Emergency Response System (PERS) services.

Hours
On call 24 hours a day, 7 days a week.
Eligibility
Must be under the care of a physician.

Intake Process
Call Intake at 716-685-4870. Can be referred by anyone. Service must be ordered by a physician, with the exception of PERS.
Program Fees
Patients will be seen regardless of economic status.
