Description
Works with health and human service providers and consumers to develop and implement efforts that focus on improving birth outcomes especially for populations who are at risk of poor outcomes. Serves as a resource for those in Erie County seeking information regarding maternal child health care services and social services for women of childbearing age and their children. Programs for women are provided in client homes.

Also offers case management provided by community health workers and family support specialists, education in parenting and child development, and a Responsible Fatherhood Program which provides a 6-week support group.

Hours
Monday-Friday, 8:30am-4:30pm.

Eligibility
New and expectant parents residing in Erie County, and women who are in need of family planning assistance.

Service Area: Erie County.
