Description
Helps communities of color to access healthcare by providing linkage to medical care and supportive services, along with one-on-one solution based counseling. Helps clients to set and meet wellness goals.

Provides sexual health education to groups and individuals and offers rapid 20 minute HIV and Hep C testing, as well as STI testing on site. Long-term prevention counseling is provided for those who are HIV positive or those at high risk of becoming infected. Issues covered include substance abuse, mental health, treatment adherence, sexual risk behavior, and linkage to other services as needed.

Serves as a Cooling Center during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.

Hours
Monday-Friday, 8:30am-4:30pm.

Eligibility
Open to individuals 18 and older from communities of color who meet screening criteria. Screening completed upon intake.

Service Area: Erie County.

Screening process handled by intake staff.
