Description
Provides a variety of different services for individuals ages 60 and older.

Services include:
- Case Management
- Home Care Aides (personal care and housekeeper aides to help in the home)
- Lifeline (Personal Emergency Response System)
- Outreach visits (visits to home-bound and frail elderly to link to necessary community services such as home delivered meals)
- Information and Assistance to support caregivers providing assistance to seniors in the community

Additional services include information, counseling, training, newsletter, support groups, health promotions, medical transportation, advocacy, nutrition counseling, minor home repairs, Health Insurance Counseling, Medicare Part D, EPIC (Elderly Pharmaceutical Insurance), Medicaid/Food Stamps, and assistance with applying for other services.

Serves as the point of contact for the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs.

Maintains NY Connects Orleans County - an Aging and Disability Resource Center that serves from birth to death with Long Term Care needs.

Labor Day to Memorial Day: Monday-Friday, 8:30am-5pm.

Memorial Day to Labor Day: Monday-Friday, 8am-4pm.

Residents of Orleans County age 60 or older.
