Group Residences for Adults With Disabilities, Semi-Independent Living Residences for Adults With Disabilities

Description
Provides a least restrictive living environment for chronically mentally ill adults. Length of stay averages 1-3 years. Residential counselors focus on increasing residents' independent daily living skills.

OMH Certified Supervised Community Residences (CR): On site staffing provided 24 hours a day, 7 days a week, relevant to the provision of community rehabilitation skills.

Supported Transitional Housing Program (available in Erie County): Offers housing in the form of apartments for seriously and persistently mentally ill adults. Candidates must be enrolled in SPOE and demonstrate that they can live independently within the community. Rental stipend and case management services are offered with this program.

OMH Certified Treatment Apartment Programs (TAP; available in Erie, Wyoming & Genesee Counties): A staff person visits these apartments 3-5 times/week relevant to provision of community rehabilitation skills. Has staff available 24 hours a day, 7 days a week.

OMH Licensed Service Enriched Single Residence Occupancy (SRO): Efficiency apartments in a supportive community. Provides case management, activities, and 3 meals a day. Has a smaller staff-to-resident ratio than a CR or a TAP.

OMH Non-Certified Supported Housing Program (available in Erie, Genesee, Orleans & Wyoming Counties): Designed to assist consumers who have a limited income with affordable housing options and connect them with pertinent community supports.

Hours
Monday-Friday, 9am-5pm.

Eligibility
Individuals 18 years and older with a mental health diagnosis, in need of supervision.

Service Area: Erie, Genesee, Orleans & Wyoming Counties.

Call for an application and other admission criteria.

Program Fees
Most applicants apply for SSI, Medicaid, or Public Assistance. Few are private pay.
