Description
Offers hospice, palliative care, and general end of life support.

Transitions Program helps patients and their families who are dealing with serious or perhaps chronic illness; this is the least medically involved of the programs offered.

Supportive Care is further along the continuum, offering some medical palliative support and can work with a patient's existing primary care physician to provide the most comprehensive pain and symptom management as well as social work support.

Hospice is the final program on the continuum, supporting patients with a prognosis of 6 months or less with a holistic and interdisciplinary model of care (including music and massage therapies when appropriate). Hospice care can be provided wherever the patient calls "home".

Hours
Monday-Friday, 9am-5pm.

Martin-Linsin Residence: 24 hours a day, 7 days a week.

Eligibility
Individuals who reside in Orleans County and are dealing with serious illness.

Intake Process
Initial referrals to Hospice can be made by family members, friends, doctor, or another health care professional. Patient needs a physician's approval to be admitted to the program; each situation is assessed on an individual basis to determine suitability for admission.
