Offers affordable quality health care. Services include: - Primary health care for adults and children, including physical exams and screening for anemia, diabetes, hypertension, obesity, cholesterol, and other conditions - Preventive health screening - Mental health referrals - Dental referrals - Hearing care - Vision care - Medication assistance - Lab and diagnostic imaging assistance - STD screenings - Women's health services, including pap tests, pelvic exams, and referrals for mammography - Insurance enrollment navigator program - Information and referral ##REENTRY##

Office: Monday-Thursday, 9am-12 noon. Medical clinics held weekly on various days and times; appointments needed.

Uninsured or underinsured individuals. No geographic restrictions. Service Area: Allegany, Genesee, Livingston, Monroe, Ontario, Orleans, Steuben, & Wyoming Counties, as well as surrounding counties.

A $5.00 donation is requested for services. A $20.00 donation is requested for employment and college physicals.
