Description
A 160 bed skilled nursing facility. Provides the following services:
- Assisted Living: Provides a supportive living environment designed to reduce daily responsibilities while maintaining comfort and routine. Residents benefit from services and activities that support engagement, safety, and overall wellbeing. As care needs change, residents have access to additional levels of support, including memory care and skilled nursing services when needed.
- Long-Term Care: Comprehensive, ongoing care designed to support individuals with long-term medical needs. Develops individualized care plans that are regularly reviewed and adjusted to ensure consistent care and coordination.
- Short-Term Care: Supports individuals recovering after hospitalization, illness, or surgery. Provides clinical oversight and a structured environment focused on recovery and safe transition.
- Dementia & Alzheimer's Care Program: Provides specialized care for individuals with Alzheimer’s disease and other memory-related conditions, with activities designed to promote engagement and daily functioning. Team members receive focused training to support cognitive needs in a structured, secure environment.
- Wound Care Program: Provided by trained clinical staff in coordination with physicians. Treatment plans are developed to support healing, monitor progress, and address individual medical needs.

Hours
Residential care services provided 24 hours a day, 7 days a week.
