Description
Provides home-bound older adults having a history of serious/chronic mental illness with a link to the appropriate medical and social services within the community. The goal is to delay or prevent hospitalization. Nurse will make house calls.
Hours
Monday-Friday, 8am-4pm.
Eligibility
Ages 50 years and older, living in Niagara County.
