Description
Required and recommended vaccines are available for children and adults, including vaccines for travel outside the U.S.
Offers tuberculin skin testing for college entrance or work requirements. Two appointments are required; one to apply the test and one to read the results, 2-3 days later.
Participates in the Vaccine for Children (VFC) Program, whereby vaccines are provided free of charge to qualifying children under 19 years of age, and the Vaccine for Adult (VFA) Program, whereby vaccines are provided free of charge to qualifying adults 19 and older. Program is for those who are uninsured or underinsured.
There is a second clinic location at Trinity Lutheran Church, 67 Saxton Street at LaGrange, Lockport, NY 14094; see separate listing for Immunization & Family Travel Clinic - Lockport.

Intake Process
By appointment only. For a complete list of vaccines offered, cost of vaccines and associated fees, or to schedule an appointment, call 716-278-1903.
For children under 18 years of age, a parent or other responsible adult must be with the child during the clinic visit. It is important that the child's immunization record is brought to the clinic appointment. If a parent is unable to bring the child, a responsible adult over the age of 18 can do so. The parent must provide the responsible adult with the child's immunization record and a signed permission slip stating who is bringing their child to clinic and that he/she has permission for their child to receive the necessary vaccines.

Program Fees
Prices vary by vaccine type. There is an administration fee and an out-of-country fee for individuals that do not reside in Niagara County.
There is no fee for vaccines provided by the VFC and VFA programs, but there is an administration fee.
For those not eligible for VFC and VFA programs, payment in the form of cash, check (made payable to Niagara County Department of Health), Visa, MasterCard, and Discover are accepted. Will also accept Blue Cross/Blue Shield and Independent Health. If unable to bill client's insurance provider, an itemized receipt will be provided for the client to submit to their insurance. Payment in full (including insurance co-payments) is to be paid at the time of the clinic appointment.

Languages
English is the primary language. Vaccine information statements are available in languages other than English. For individuals who don't speak English, it's recommended to bring an interpreter to the appointment.
