Central Intake/Assessment for Mental Health Services

Description
Program is for individuals, families, and service providers seeking Assertive Community Treatment (ACT), care management, residential or housing services for adults and children living with mental health illness/emotional disturbances. Assesses the strengths and needs of those referred and then creates a plan for linkage with the most appropriate services. Coordinates with Health Homes for most appropriate Care Management service as appropriate.

Hours
Monday-Friday, 8:30am-4:30pm.

Eligibility
Children 5-21 years old; adults 18 years and older.

Service Area: Niagara County.

Intake Process
Call for information; eligibility and level of service are determined by application.

Free.
