Adult Residential Care Homes, Assisted Living Facilities, Secured Supportive Housing Units
Description
Assisted living facility offering individualized programs, including the following:
- Adult Home (AH): Provides on-site monitoring, case management, food services, and personal care and/or home care services (either directly or indirectly) in the least restrictive and most home-like setting consistent with the resident's preferences and physical and mental status.
- Assisted Living Program (ALP): Serves people who are medically eligible for nursing home placement but prefer a less medically intensive, lower cost setting. Provides personal care, room, board, housekeeping, supervision, home health aides, personal emergency response services, nursing, physical therapy, occupational therapy, speech therapy, medical supplies and equipment, adult day health care, a range of home health services, and the case management services of a registered professional nurse.
- Special Needs Assisted Living Residence (SNALR): Provides a secured environment with supportive therapeutic activities for people with Alzheimer's or other types of memory impairments. Offers both individual and group sensory programs for residents in the memory care program as well as Validation Therapy for a more holistic approach to treating memory disorders. Programs are administered by Alzheimer's certified staff.

Facility services provided 24 hours a day, 7 days a week.
Eligibility
Seniors in need of assisted living; specific requirements vary by program.
To be eligible for Assisted Living Program (ALP), both Medicaid recipients and private-payers must be medically eligible for, and would otherwise require, placement in a nursing home due to the lack of a home or suitable home environment. However, eligible ALP residents must not require continual nursing care, be chronically bedfast, or be impaired to the degree that they endanger the safety of other ALP residents.
