Description
Provides hospice care.

Niagara Hospice is dedicated to providing the right care, in the right place, at the right time. Hospice care is appropriate for people with any advanced stage illness, not just cancer, where the prognosis is a life expectancy of approximately six months or less if the illness runs its normal course (strictly Medicaid recipients can receive care up to 12 months under the New York State hospice regulation). Some patients receive hospice care beyond six months. Care is given by an interdisciplinary team of skilled professionals and trained volunteers. Team includes a hospice physician, primary MD, nurse, social worker, pharmacist, spiritual care and bereavement counselor, hospice aide, and homemaker.

Niagara Hospice House provides services to Niagara Hospice patients and their families who are not able to receive home hospice care. Residential wing consists of 16 suites specifically designed for hospice patients in need of long term care. The inpatient wing has 10 suites designed for patients needing short-term medical monitoring and symptom management.

The Bereavement Department offers grief support to families in a variety of ways, available for 13 months after the passing of a loved one:
- Supportive telephone calls and mailings
- Individual and family grief counseling
- Bereavement workshops and groups for adults
- Memorial events throughout the year
- Resource materials on grief and loss
- Information and referral services
- E-newsletters

Hours
Office: Monday-Friday, 8:30am-4:30pm.

On-call staff available to current patients 24 hours a day, 7 days a week.

Eligibility
Anyone can make a referral including self-referrals from patients. Niagara Hospice provides care to Niagara County residents whose prognosis is approximately six months or less, if the illness runs its normal course, regardless of diagnosis. Many patients live beyond 6 months due to comfort measures Niagara Hospice can provide. If curative treatment is no longer effective and you desire a treatment plan based on comfort and dignity, hospice care may be appropriate.

Intake Process
Call Intake Department. Individuals need not try to determine if they qualify - intake representatives are available to provide an informational consultation at no cost and with no obligation to accept services.

Program Fees
No one is denied hospice care due to inability to pay. Medicare Part B and Medicaid cover hospice care, as do most private insurers.

Will attempt to find an interpreter if needed.
